If you are applying for financial aid, do not complete any of these forms until you first review the application procedures in the Apply for Aid section of this website AND begin Step 1 by submitting our main online institutional financial aid form on "MyHLS Financial Aid" (see "Login to MyHLS Financial Aid" button below the lower right navigation bar). Below you will find a comprehensive list of all of our application and transaction forms. You will generally not need to submit all of these forms. You should refer to the "Documents" tab of "MyHLS Financial Aid" after submitting the main application for a customized list of documents you must submit to complete your application for aid.
HLS Student Financial Services is a paperless office. We do not maintain student financial aid records in hard copy format. All "paper" documents, including all required Federal tax data, will be imaged, stored and managed electronically. It is extremely important that you carefully read through the information found on our Document Submission & Imaging web page before you begin to submit any of your required tax documentation to HLS Student Financial Services. The application process for 2014-2015 is closed and all forms have been removed from our website so that we can begin the process of updating everything for the next application cycle. However, if you need to apply for financial aid for 2014-2015, you can still do so. Please email Student Financial Services and someone will get back to you with assistance.
The application process for 2015-2016 has not yet begun. All forms are in the process of being updated for the upcoming application cycle. We expect to open the process for admitted students the week of February 16, 2015 and late-March 2015 for continuing students.
