Financial Aid Forms, & Loan Signing Directions

General Form Information

If you are applying for financial aid, do not complete any of these forms until you first review the application procedures in the Apply for Aid section of this website AND begin Step 1 by submitting our main online institutional financial aid form on "MyHLS Financial Aid" (see "Login to MyHLS Financial Aid" button below the lower right navigation bar). 

  • Below you will find a comprehensive list of all of our application and transaction forms. You will generally not need to submit all of these forms. You should refer to the "Documents" tab of "MyHLS Financial Aid" after submitting the main application for a customized list of documents you must submit to complete your application for aid.
  • You should also review the Document Descriptions section of our web site for detailed information about all of the documents below, why they are required, and helpful hints on completing them successfully. 

SFS is a Paperless Office

 HLS Student Financial Services is a paperless office. We do not maintain student financial aid records in hard copy format. All "paper" documents, including all required Federal tax data, will be imaged, stored and managed electronically. It is extremely important that you carefully read through the information found on our Document Submission & Imaging web page before you begin to submit any of your required tax documentation to HLS Student Financial Services. For your convenience here is a summary of the two most efficient and secure ways to submit your "paper" document's to us.

  1. Upload your data directly into our system using our Secure File Upload service. (Will only accept PDF formats)
  2. If you are unable to use the Secure File Upload service, send us your documents via Fax being careful to complete a Document Fax Cover Sheet to go with the documents.

Secure Web Forms

2014-2015 Financial Aid Application Forms (Student)

The following are secure web forms to be completed by the student applicant. Data transmitted via these forms is always encrypted and protected. The correct username and password needed to access these forms is the same username and password you use to access other HLS network and web services including HLS Email.

  • Username: Your username is the part of your HLS email address which comes before the "@" symbol plus the JD class designation tag (i.e. .jd10, jd11, jd12, etc.). For example, if your email address is "jsmith@jd10.law.harvard.edu," then your username would be "jsmith.jd10".
  • Password: Your password is the same password that you use to login to your HLS email account.
  • ONLY SUBMIT FORMS WHICH SHOW AS REQUIRED ON YOUR CUSTOMIZED DOCUMENT LIST ON "MY HLS FINANCIAL AID"

2014-2015 Financial Aid Application Forms (Parent)

The following are secure web forms to be completed by the applicant's parents. Data transmitted via these forms is always encrypted and protected; however these forms do not require log in. Parents can simply proceed directly to the form and submit their information.

  • Parents should be careful to enter the student's Official HLS email address on each submission. This is how we will match your data up to the student's electronic file.
  • ONLY SUBMIT FORMS WHICH SHOW AS REQUIRED ON THE STUDENT'S CUSTOMIZED DOCUMENT LIST ON "MY HLS FINANCIAL AID"

2013-2014 Financial Aid Application Forms (Both Student and Parent Forms)

The application process for 2013-2014 is closed and all forms have been removed from our website so that we can begin the process of updating everything for the next application cycle. However, if you need to apply for financial aid for 2013-2014, you can still do so.  Please email Student Financial Services and someone will get back to you with assistance.
