If you are applying for financial aid, do not complete any of these forms until you first review the application procedures in the Apply for Aid section of this website AND begin Step 1 by submitting our main online institutional financial aid form on "MyHLS Financial Aid" (see login box in the lower right navigation bar).
Below you will find a comprehensive list of all of our application and transaction forms. You will not need to submit all of these forms. You should refer to the "Documents" tab of "MyHLS Financial Aid" after submitting the main application for a customized list of documents you must submit to complete your application for aid.
You should also review the Document Descriptions section of our web site for detailed information about all of the documents below, why they are required, and helpful hints on completing them successfully.
Finally, please note that HLS Student Financial Services is a paperless office. We do not maintain student financial aid records in hard copy format. All paper tax forms, PDF's, and any other miscellaneous documents submitted will be automatically imaged, stored and managed electronically. It is extremely important that you carefully read through the information found on our Document Submission & Imaging web page before you begin to complete/submit any of your required documentation to HLS Student Financial Services.
When submitting any required tax data (1040, 1065,1120,1120S, W-2s, Schedules, K-1s, etc.) or any other type of information in hard copy, we require you to include a type-written cover sheet found via the link below.
Open the PDF and complete it electronically following the instructions provided on the document. Once completed, print it out and attach it to the first page of the packet of documents you are preparing to submit. After electronically completing the cover sheet, we strongly recommend that you FAX your packet of documents to our office. If FAX is not convenient, the next best thing is to send your documents electronically via EMAIL.
Sending your documents via FAX or EMAIL ensures that your data gets matched quickly to the correct student's electronic records in our document management system without requiring us to manually scan your documents.

The following are secure web forms to be completed by the student applicant. Data transmitted via these forms is always encrypted and protected. The correct username and password needed to access these forms is the same username and password you use to access other HLS network and web services including HLS Email.
The following are secure web forms to be completed by the applicant's parents. Data transmitted via these forms is always encrypted and protected; however these froms do not require log in. Parents can simply proceed directly to the form and submit their information.
